Hello Wound Care Connection! For those of you who have patients with ostomies, this new tool is AWESOME!!! They can click the link and trouble shoot their common skin problems with instruction for management. Take a look!!
The Peristomal Skin Assessment Guide for Consumers is an online tool that provides basic guidance for people living with an ostomy. It is designed to help identify common skin problems, provide next steps for care or management and appropriately refer individuals to seek care or support from a WOC nurse or a NSWOC for additional support.

| Chronic obstructive pulmonary disease (COPD) is one of the top diagnoses involved with readmissions throughout Minnesota. Lake Superior QIN, Minnesota Hospital Association, and the American Lung Association Minnesota Chapter are partnering to provide a webinar series to inform providers across the continuum of care in support of patients with COPD. Pulmonary Rehabilitation is one key element to successful treatment and care for patients with COPD. There are many options available throughout the state of Minnesota, and we will hear from one featured program at CentraCare St. Cloud. During the session, you will learn what pulmonary rehabilitation is, who benefits from it, and how it supports the family and friends of the person with COPD as well as the outcomes and benefits from the completion of a pulmonary rehabilitation program. |
